Please submit this form with annual dues of $30 at a Guild meeting.  The Guild's fiscal year is July 1st through June 30th.  Dues are prorated to $15 after January 1st for new members only. Membership cards are distributed at the Guild Meetings when you pay your dues. This website does not collect dues online.
